Eastern Mavericks District Basketball Club Payment Plan Request Form

$20 Admin Fee will apply.

Request form to set up a payment plan for Championship Season subscription fees.

Please submit a separte request for each player. Payment plans can only be set up separately for each invoice.

First payment must be made prior to the first game to enable player to be dropped into their team in BasketballConnect.

System contstraints prevent the start date of the plan to be adjusted. The date is preset. This may require payment plan applicants to make the first payment before the due date of the first instalment.

All requests for payment plans must be submitted by 15/01/2026
