Event Waiver

Eastern Mavericks Holiday Camp – Participant Waiver

I, the undersigned, acknowledge that participation in the Eastern Mavericks Holiday Camp involves physical activity and carries a risk of injury.

By registering my child, I acknowledge and agree that:

• My child is medically fit to participate in basketball activities and physical exercise.
• I understand that basketball activities may involve running, jumping, physical contact and other activities where injury may occur.
• I accept these risks and release the Eastern Mavericks, its coaches, volunteers, staff and representatives from liability for any injury, loss or damage that may occur during participation in the camp, except where required by law.

I also acknowledge that:

• My child will follow the instructions of coaches and staff at all times.
• The organisers reserve the right to remove a participant if behaviour is unsafe or inappropriate.
• In the event of an injury or medical emergency, I authorise event staff to seek appropriate medical treatment if I cannot be contacted immediately.

Media Consent

I give permission for photographs or video taken during the camp to be used by the Eastern Mavericks for promotional, marketing or social media purposes.

I confirm that I have read the above Event Waiver in its entirety.*
