Eastern Mavericks Parent Feedback Survey

At the Eastern Mavericks, we are committed to continually improving and progressing as a club. We strive to provide the best possible experience for our players, families, coaches and volunteers, and we value feedback that helps us identify both our strengths and opportunities for growth. Please complete one survey per child playing or training within the club.

This survey is anonymous and is designed to gather constructive feedback that will assist the club in planning for the future and enhancing the experiences of our members. We encourage responses that are respectful, specific and focused on improvement.

If you would like a member of the club to contact you regarding your feedback, please provide your name and contact details at the end of the survey. Without this information, we may be unable to seek clarification or discuss the matter further.

Please note that, due to the anonymous nature of this survey, not all feedback can be individually addressed. In particular, concerns relating to historical matters or issues that have not previously been raised through the appropriate club processes may not be able to be investigated or resolved through this survey.

This survey is not intended to replace the club's formal policies and procedures. Matters requiring investigation, immediate attention or a formal response should continue to be raised through the appropriate channels as they occur.
